Diabetes mellitus (DM) is a chronic metabolic disorder caused by the pancreas not being able to produce enough insulin. This disease is caused by factors of age, obesity, and family history. Impaired skin integrity in patients with type II diabetes mellitus will cause nursing problems that can cause wounds on the feet that are blackish red and foul smelling due to blockages that occur in medium or large blood vessels in the legs. Type II diabetes mellitus patient with skin integrity problems. The design of this case study is descriptive analytic in the form of a case study to explore the problem of nursing implementation of type II diabetes mellitus patients with skin integrity problems at Bhayangkara Moh Hasan Hospital in 2021. In this case study, there are two types of data analysis, namely subjective data and objective data. . The results of the case studies show that the implementation of nursing for type II diabetes mellitus patients with problems with skin integrity disorders is wound care, skin care education (diabetic wound).The two implementations can overcome the problem of skin integrity disorders in diabetes mellitus patients and most of them are in accordance with the action plan that has been implemented. As for the advice given, it is hoped that diabetes mellitus sufferers of the importance of wound care to support the health of the patient's foot healing.
