Tujuan penulisan laporan ini adalah untuk memberikan asuhan kebidanan komprehensif dengan pendekatan manajemen kebidanan dan pendokumnetasian dalam bentuk SOAP pada Ny. “Y” di Tempat Praktik Mandiri Bidan (TPMB) Lismarini Kota Palembang. Metode penulisan laporan tugas akhir ini menggunakan metode studi penelaahan kasus dengan sasaran Ny. “Y” umur 29 tahun anak kedua, tidak pernah keguguran. Data yang digunakan dalam pengumpulan data ialah data primer dan data sekunder melalui wawancara (anamnesis), pemeriksaan fisik dan pemeriksaan penunjang. Hasil pembahasan menunjukan pada Ny. “Y” G2P1A0 dilakukan relaksasi Hypnobirthing bertujuan agar dapat membantu ibu mengurangi rasa cemas dan lebih rileks dalam menghadapi persalinannya. Pada masa persalinan Ny”Y” mengalami ketuban pecah dini dan pembukaan tidak bertambah sehingga Ibu dilakukan tindakan Sectio Caesaria (SC) pada usia kehamilan 40 minggu. Melalui laporan ini, di harapkan bagi tenaga kesehatan dapat meningkatkan mutu dan kualitas kebidanan komprehensif sesuai standar pelayanan yang di tetapkan.
