Tujuan Laporan ini adalah mampu memberikan asuhan kebidanan komprehensif pada Ny.”N” mulai dari kehamilan, bersalin, bayi baru lahir dan nifas, postpartum dengan cara melakukan pengkajian dengan menggunakan pendokumentasian SOAP. Plak bisa dibersihkan secara mekanis maupun kimiawi. Salah satu cara untuk membersihkan plak adalah dengan cara mengkonsumsi buah apel, karena apel mengandung tannin yang bersifat sebagai pengelat (astringent) yang bersifat spasmolotik dan sebagai antiseptic. Dan melalui kandungan seratnya yang dapat membersihkan sisa plak gigi dengan cara menggigit dan mengunyah menjadikan buah apel mempunyai daya self cleansing. Dalam penelitian ini, penulis mencoba meneliti Buah Apel Sebagai Self Cleansing Terhadap Penurunan Skor Plak yang dilakukan di MI Tarbiyah Islamiyah Palembang Tahun 2018. Metode penelitian yang digunakan dalam penelitian ini adalah survei analitik. Adapun sampel penelitian ini sama yaitu 60 orang yang terdiri dari 30 orang mengkonsumsi buah apel dan 30 orang tidak mengkonsumsi buah apel dengan jumlah populasi sebanyak 189 orang. Teknik pengambilan sampel diambil dari populasi Non Random dengan menggunakan teknik Purposive Sampling, yaitu didasarkan pada suatu pertimbangan tertentu yang dibuat oleh peneliti sendiri berdasarkan ciri atau sifat-sifat populasi yang sudah diketahui sebelumnya, dan analisa data yang digunakan adalah analisa data Bivariat dengan pengujian statistik Chi Square dengan uji Kolmogorov Smirnov Z menunjukan hasil P=0,000, yang berarti terdapat hubungan yang signifikan (p<0,05) antara mengkonsumsi buah apel terhadap penurunan skor plak. Hipertensi sering disebut sebagai the silent killer karena penderita tidak dapat merasakan gejalanya. Penelitian ini bertujuan untuk mengetahui gambaran asupan zat gizi makro, zat gizi mikro, serat, status gizi dan tingkat pengetahuan gizi pada pasien hipertensi di Puskesmas Alang-Alang Lebar Palembang. Jenis penelitian ini adalah deskriptif dengan desain penelitian cross sectional dan teknik pengambilan sampel purposive sampling. Sampel dalam penelitian ini yaitu 45 orang. Data primer yaitu, pengukuran tekanan darah, tinggi badan ,berat badan, data asupan bahan makanan dan tingkat pengetahuan gizi gizi dan data sekunder yaitu, profil Puskesmas Alang-Alang Lebar Palembang. Hasil penelitian menunjukkan bahwa sebagian besar asupan energi baik 51,1%, asupan protein lebih 84,4%, asupan lemak lebih 57,8%, asupan karbohidrat baik 68,9%, asupan natrium lebih 80%, asupan kalium rendah 95,6%, asupan magnesium rendah 88,9%, asupan vitamin C baik 82,2%, asupan serat kurang 93,3%, status gizi lebih 53,3%, tingkat pengetahuan gizi baik 88,9% dan tingkat hipertensi sebagian besar derajat I 60,0%. Disarankan bagi pasien untuk memperhatikan asupan protein, lemak, natrium, kalium, magnesium, serat, dan status gizi agar tekanan darah menjadi normal. Pada akhir 2019 terjadi penyebaran virus Corona. Pandemi Covid-19 ini mempengaruhi jumlah kunjungan pasien poli gigi. Jumlah kunjungan pasien poli gigi sebelum masa pandemi memiliki angka yang cukup tinggi sedangkan jumlah kunjungan pasien poli gigi pada masa pandemi mengalami penurunan yang cukup drastis. Maka dari itu penelitian ini bertujuan untuk mengetahui apakah kunjungan di Puskesmas Gelumbang mengalami penurunan atau mengalami peningkatan. Tujuan: Untuk mengetahui gambaran kunjungan pasien poli gigi sebelum dan semasa pandemi Covid-19 di Puskesmas Gelumbang Tahun 2018-2020. Metode: Metode penelitian yang digunakan dalam penelitian ini adalah metode deskriptif analitik. Subjek penelitian yaitu buku registrasi pada tahun 2018-2020. Hasil: Menunjukkan bahwa terjadi penurunan kunjungan pasien yang cukup drastis yaitu tahun 2018 sebanyak 649 , tahun 2019 sebanyak 1110, dan tahun 2020 sebanyak 473. Kunjungan pasien perempuan 1.289 dan laki-laki 943 pasien. Kunjungan tertinggi rentang umur dibawah 14 tahun sebanyak 911. Penyakit tunggal terbanyak yaitu pulpitis 597. Tindakan terbanyak yaitu tindakan pencabutan 54. Kesimpulan:Berdasarkan hasil penelitian ini dapat disimpulkan bahwa gambaran kunjungan pasien poli gigi sebelum dan semasa pandemi Covid-19 di Puskesmas Gelumbang Tahun 2018-2020 terjadi penurunan yang drastis dimana pasien perempuan lebih banyak daripada laki-laki dan kebanyak pasien dibawah umur 14 tahun. Pulmonary tuberculosis (TB) is an infectious disease caused by the bacterium Mycobacterium tuberculosis that attacks the lung parenchyma characterized by the formation of granulomas. Ineffective airway clearance in pulmonary tuberculosis patients will cause nursing problems that can cause respiratory function disorders. This case study aims to describe the implementation of nursing implementation in pulmonary tuberculosis patients with ineffective airway clearance problems. The case study subjects studied were at least two people with the same nursing problem, namely the problem of ineffective airway clearance. The design of this case study is descriptive in the form of a case study to explore the problem of nursing implementation of pulmonary tuberculosis patients with ineffective airway clearance problems at the Palembang Bari Hospital in 2022. In this case study, there are two types of data analysis used, namely subjective data and objective data. The results of the case study show that the implementation of nursing for pulmonary tuberculosis patients with ineffective airway clearance problems is to adjust the semi-Fowler position, assess respiratory function and practice effective coughing. Of the three implementations that have been carried out, it can overcome the problem of ineffective street cleaning in pulmonary tuberculosis patients and most of them are in accordance with the action plan that has been implemented. The suggestions given are expected to make people with pulmonary tuberculosis aware of the importance of maintaining good health in the environment and also healthy living behavior to support health. Tujuan studi kasus adalah mendapatkan gambaran implementasi keperawatan pada pasien thyfoid dengan masalah hipertermi di ruang manggaris RSUD Sekayu. Jenis penelitian ini adalah deskriptif analitik analitik dengan pendekatan implementasi keperawatan. Subyek penelitian yang digunakan adalah dua pasien thyfoid dengan masalah hipertermi, studi kasus ini dilakukan pada tanggal 06-08 juni 2018. Dari hasil pengkajian kedua pasien mengatakan badan terasa panas dan lemas. Pada tanggal 04 juni 2018 dilakukan pemeriksaan laboratorium tes uji widal dengan hasil pasien I +1/160 dan pasien II +1/320. Terdapat empat diagnosa keperawatan yang ditemukan tetapi menjadi focus m alah pada studi kasus ini adalah hipertermi berhubungan dengan proses inflamasi. Intervensi yang dibuat sesuai dengan teori serta kondisi dan situasi pasien. Implementasi yang dilakukan yaitu mengkaji tanda-tanda vital, memberikan kompres hangat, mengkaji ketepatan jenis pakaian yang digunakan, dan menganjurkan pasien banyak minum. Tindakan kompres hangat yang dilakukan pada kedua pasien efektif untuk menurunkan panas karena dapat mempengaruhi tubuh dengan vasodilatasi pembuluh darah dimana menyebabkan pembuangan energy panas melalui kulit. Focus masalah keperawatan yang muncul dapat teratasi dalam masa perawatan 3 hari. Diharapkan perawat dapat meningkatkan kualitas implementasi secara komprehensif baik secara mandiri, maupun kolaborasi dan melakukan pendokumentasian implementasi keperawatan dengan baik.
