Hasil: Setelah dilakukan implementasi keperawatan keluarga yang sama terhadap kedua klien selama 3 hari kesimpulan yang didapat yaitu kedua klien sudah mampu melaksanakan latihan batuk efektif dan pernapasan kembali normal dan kedua klien mampu menerapkan latihan batuk efektif dan pendidikan kesehatan mengenaipenyakit tuberculosis paru. Jadi Apoteker di Puskesmas Kota Palmebnag belum memiliki persepsi yang baik tentang Pharmaceutical Care. Kesimpulan : Apoteker telah melaksanakan Pharmacutical Care pada penyakit hipertensi di puskesmas Kota Palembang, namun belum memiliki persepsi yang baik yang sesuai dengan pedoman Pharmaceutical Care untuk penyakit hipertensi. Prevalensi di Indonesia 43,2% (2012) menjadi 45,9% (2013), sedangkan prevalensi Sumatera Selatan 30,33% (2016) dan di Kota Palembang 23% (2017), sedangkan di Puskesmas Karya Jaya tersebut sebanyak 806 orang (2021). Gaya hidup tidak sehat yaitu kebiasaan konsumsi tinggi natrium, rendah kalium, pola makan yang tidak sehat, jenis kelamin dan status gizi IMT adalah faktor terjadinya penyakit hipertensi. Tujuan dari penelitian adalah untuk mengetahui gambaran asupan natrium, kalium, pola makan, pada pasien hipertensi rawat jalan di Puskesmas Karya Kota Palembang. Jenis penelitian adalah penelitian deskriptif dengan desain penelitian cross-sectional. Populasi penelitian ini semua penderita hipertensi yang berkunjung dan menjalani pengobatan di Puskesmas Karya Jaya Kota Palembang yang diambil menggunakan teknik accidental sampling. Sampel penelitian berjumlah 46 orang. Pengumpulan data konsumsi makanan tinggi natrium menggunakan recall dan food frequency quesionaire (FFQ). Dan status gizi diperoleh dengan cara pengukuran. Data analisa secara univariat dan biivariat. Hasil penelitian menunjukkan responden yang menderita hipertensi memiliki kebiasaan konsumsi makanan tinggi natrim yaitu 24 orang (52%), responden yang mengkonsumsi asupan kalium rendah sebanyak 46 orang (100%), responden yang berjenis kelamin laki-laki sebanyak 36 orang (78,2%) dengan penyakit hipertensi dan status gizi pada responden dikategorikan gizi normal sebanyak 41 (89%). The purpose of this study was to carry out and determine the description of nursing care in patients with Type Diabetes Mellitus II with nutrient deficit problems. The type of research used is descriptive analytic in the form of a nursing care approach which includes assessment, nursing diagnosis, planning, implementation, and evaluation. The research subjects were two diabetes mellitus patients with nutritional deficits. The research was conducted on April 5 2021 until April 8 2021, at the Siti Fatimah Regional Hospital, South Sumatra Province in 2021. Based on the results of the study conducted on both patients for 3 days to overcome nutritional deficits, it was found that nausea was reduced. In addition, after being given education about the DM diet, the two patients were able to find out how to do the right DM diet. Mirror Therapy is used to treat muscle weakness in the rehabilitation phase of Stroke. Research Objectives: To find out the description of the application of Mirror Therapy to increase muscle strength, it is obtained an overview of the application of Mirror Therapy to increase muscle strength of patients with Stroke in the Rehabilitation Phase. Research Methods: This study uses a case study method using a nursing research process approach to two patients as research respondents. Results: The study found that two patients experienced the same symptoms, namely an increase in muscle grade after the application of mirror therapy, the results obtained were a muscle strength scale namely 4.
