Pasien ± 1 bulan yang lalu sebelum MRS pasien mengeluh sesak pada bagian dada seperti terjadi penyempitan, batuk (+), dahak (-), berat badan dan nafsu makan menurun, badan lemas, mual, keluar keringat berlebihan pada malam hari, sulit tidur. Pengkajian yang dilakukan pada pasien dua (Tn. BI) berjenis kelamin laki-laki dilahirkan di Palembang pada tanggal 09 Maret 1977. Pasien + 3 hari yang lalu pasien mengeluh nafasnya semakin sesak, batuk (+), dahak (-), mual, muntah, berat badan dan nafsu makan menurun, badan lemas. Diagnosa yang muncul yaitu: Ketidakefektifan bersihan jalan nafas, Gangguan pertukaran gas berhubungan dengan kerusakan membran alveolar, ketidak seimbangan nutrisi kurang dari kebutuhan tubuh berhubungan dengan mual muntah dan intoleransi aktivitas berhubungan dengan kelemahan tubuh. Perencanaan keperawatan bertujuan untuk mengatasi masalah yang timbul pada pasien. Implementasi yang dilakukan kepada pasien antara lain: mengukur tanda-tanda vital, mengkaji bunyi nafas dan pola nafas, memberikan posisi semi fowler/fowler, dan kolaborasi dalam pemberian oksigen tambahan, menganjurkan pasien melakukan oral hygine, menganjurkan pasien makan dalam porsi sedikit tapi sering, menganjurkan keluarga untuk membantu aktivitas pasien. Pada evaluasi dilakukan dengan pendekatan SOAP, evaluasi didapatkan dua masalah yang dapat teratasi sebagian yaitu gangguan pertukaran berhubungan dengan kerusakan membran alveolar dan intoleransi aktivitas dan masalah yang dapat teratasi yaitu ketidak seimbangan nutrisi berhubungan dengan mual muntah. Sedangkan jumlah kematian pasien rawat inap RSUD Palembang BARI dalam tiga tahun terakhir sebanyak 37 orang tahun 2016, meningkat pada tahun 2017 sebanyak 45 kematian dan menurun pada tahun 2018 sebanyak 16 kematian. Metode :Penulis menggunakan metode deskripsi, adapun sampelnya adalah klien M dan H, data ini diperoleh dengan cara yaitu : wawancara, pemeriksaan, observasi aktivitas, memperoleh catatan dan laporan diagnostik, bekerjasama dengan teman sekerja. Sehingga menyebabkan masalah ketidakefektifan bersihan jalan napas yang ditandai dengan batuk-batuk dan penumpukan sputum di jalan napas, pola napas tidak efektif dan penurunan berat badan. Implementasi keperawatan yang dapat dilakukan mengatur posisi semi fowler, latihan batuk efektif dan tarik napas dalam, kolaborasi dalam pemberian obat. Desain penelitian ini menggunakan metode deskriptif dalam bentuk studi kasus dengan melakukan asuhan keperawatan meliputi Pengkajian, diagnosa, intervensi, implementasi, dan evaluasi dengan memfokuskan implementasi pada penerapan masalah ketidakefektifan bersihan jalan napas. Adapun subyek penelitian studi kasus yang diteliti sebanyak 2 pasien yang di rawat di ruang Baung Rumah Sakit Khusus Paru Sumatera Selatan. Hasil penelitian menunjukkan bahwa implementasi Keperawatan Tuberkulosis Paru Dengan Masalah Ketidakefektifan Bersihan Jalan Napas yang diberikan kepada pasien mampu mengatasi berkurangnya sputum yang ada pada jalan napas.
