Bronchial asthma is a non-communicable disease caused by chronic inflammation of the airways that causes hyperresponsiveness, mucus production, and mucosal edema. This causes recurrent symptoms such as dyspnea, wheezing, heaviness in the chest, and coughing, especially at night. The general objective is to describe the implementation of nursing for bronchial asthma patients with ineffective airway clearance problems. The design used is descriptive in the form of a case study, the samples taken are two people with the same medical diagnosis, namely bronchial asthma with ineffective airway clearance problems, at Bhayangkara Hospital Palembang. The results showed that client 1 and client 2 were obtained. The results showed that the implementation of nursing in bronchial asthma patients with ineffective airway clearance problems given to patients was able to reduce shortness of breath in patients. The conclusion of the study is that after implementing nursing in bronchial asthma patients with ineffective airway clearance problems, namely practicing effective coughing, adjusting the semi-fowler position, and giving warm drinks from both patients, it can be concluded that the three implementations can make the patient comfortable and can reduce the patient's shortness of breath. Subyek penelitian yang diteliti berjumlah dua pasien yaitu bayi dengan Berat Badan Lahir Rendah dengan Masalah Keperawatan yang sama. Penelitian dilakukan pada tanggal 09 Juni – 13 Juni 2018 dengan melakukan wawancara dengan orang tua pasien dan pemeriksaan fisik. Hasil pengkajian didapatkan Bayi Ny. M memiliki berat badan lahir 2000 gram, sedangkan bayi Ny. I memiliki berat badan lahir 1900 gram, bayi lahir dismature (38 minggu). Pada tahap implementasi ketidakseimbangan nutrisi yang dilakukan yaitu mengkaji reflex hisap dan menelan, memberikan ASI secara langsung dan menimbang kenaikan berat badan bayi. Hasil: Setelah dilakukan tidakan keperawatan didapatkan hasil Adanya kenaikan berat badan dari kedua bayi. Pada bayi I dari 2000 gram menjadi 2100 gram dan pada bayi 2 dari berat badan 1900 menjadi 2180 gram. Methods: The design of this study used a descriptive method. in the form of a case study with a nursing process approach consisting of assessment, nursing diagnosis, planning, implementation, evaluation and documentation. This case study focuses on the implementation of Tissue Integrity Disorders teaching wound care and providing health education about Diabetes Mellitus which is carried out once a day in the morning. The case study subjects were 2 patients with Type II Diabetes Mellitus with problems with tissue integrity disorders in the Ahmad Dahlan Room, Muhammadiyah Hospital Palembang and started on March 11, 2022.Results: The results showed that the provision of wound nursing actions had an effective effect on reducing the infection experienced by the patient, so that the patient could carry out activities as usual and could increase the patient's knowledge about Type II Diabetes Mellitus. Diabetes Mellitus is a disease that requires good nutritional management, non-adherence to diet and eating patterns will result in instability of glucose in the blood. Methods: This research design uses a case study design with a descriptive method with a nursing care approach that includes assessment, diagnosis nursing, planning, implementation, evaluation and documentation. The case study subjects in this study amounted to two cases of Type II Diabetes Mellitus patients with the same nursing problem, namely Nutritional Deficit. Results: Based on the results of the study conducted on patient 1 who was 39 years old with a body weight of 47 kg and BMI 18,3 and in patient 2 who was 67 years old with a weight of 48 kg and BMI 18,7, it was found that there was a problem of nutritional deficits in both patients with Type II DM medical diagnosis. After the implementation was given in the form of identification of nutritional status (BMI), oral hygiene measures, and DM diet education, it was found that the problem of nutritional deficits in patients was resolved marked by an increase in weight and BMI during nursing implementation. Conclusion: Implementation in the form of identification of nutritional status (BMI), oral hygiene measures, and DM diet education in both patients have been carried out in the research field.
