Subyek studi kasus yang digunakan dalam studi keperawatan ini adalah penderita Artritis Reumathoid yang berfokus pada nyeri kronis. Studi kasus ini telah dilakukan pada tanggal 26 juni 2018 di wilayah kerja Puskesmas Talang Aur Kabupaten Ogan Ilir, pada kedua klien ditemukan 1 diagnosa yaitu nyeri kronis.Adapun implementasi yang penulis lakukan untuk mengatasi nyeri kronis pada kedua klien yaitu dengan mengajarkan klien cara melakukan teknik relaksasi nafas dalam serta penulis juga melakukan pendidikan kesehatan tentang penyakir Artritis Reumatoid, dan nyeri kronis yang dirasakan kedua klien teratasi pada hari ke 3.Diharapkan dengan adanya penulisan ini perawat mampu menerapkan ilmu dan kiatnya dengan baik dan benar, dapat meningkatkan kualitas pelayanan Puskesmas dan upaya pendidikan kesehatan terhadap keluarga, serta dapat menjadi salah satu bahan referensi dalam keperawatan komunitas dan aplikatif dalam penelitian selanjutnya. Hasil penelitian menunjukan dukungan sosial keluarga yang banyak diberikan oleh keluarga kepada penderita Gastritis adalah keluarga terlihat sangat antusias terhadap edukasi yang diberikan. Dan implementasi yang dilakukan sesuai perencanaan yang telah disusun untuk mengatasi masalah yang dialami dan melibatkan klien dan keluarga. Implementasi keperawatan yang dilakukan pada Ny.E dan Ny.Y yaitu mengkaji nyeri, mengajarkan teknik relaksasi nafas dalam dan pemberian kompres hangat. Pendekatan yang digunakan meliputi pengkajian, diagnosis keperawatan, perencanaan, pelaksanaan, dan evaluasi. Subjek penelitian yang diteliti berjumlah dua keluarga dengan kasus masalah keperawatan yaitu klien dengan pasca stroke. Studi kasus ini dilakukan di wilayah kerja Puskesmas 23 Ilir Palembang. Data pengkajian didapatkan melalui kunjungan yang dilaksanakan pada tanggal 20-25 Mei 2019. Dari data pengkajian keluarga Tn.U merupakan keluarga inti yang terdiri dari ayah,ibu,dan anak. Sedangkan keluarga Tn.A merupakan keluarga inti yang terdiri dari ayah,ibu,dan anak. Tn.U mengalami kelumpuhan pada tangan dan kaki sebelah kanan, dan Ny.Y tangan kanannya sulit untuk digerakkan. Saat dilakukan perumusan diagnosa keperawatan ditemukan 2 diagnosa yaitu hambatan mobilitas fisik,dan resiko stroke berulang, sedangkan Tn.U hambatan mobilitas fisik, dan resiko cidera. Perencanaan keperawatan bertujuan untuk mengatasi masalah yang timbul pada klien dan keluarga. Implementasi keperawatan yang dilakukan pada keluarga Tn.U dan keluarga Tn.A antara lain menjelaskan tentang stroke, tanda dan gejala, pasca stroke,dan terapinya, mobilisasi dan stroke berulang serta melakukan terapi ROM. Setelah dilakukan evaluasi keperawatan penulis mendapatkan semua masalah sudah dapat teratasi dari kedua diagnosa yang muncul, klien dan keluarga sudah mengetahui dan mengerti pendidikan kesehatn yang telah diberikan termasuk melakukan latihan ROM secara mandiri. Berdasarkan hasil asuhan keperawatan dan pembahasaan, maka saran yang bisa disampaikan agar keluarga serta masyarakat dapat menanggulangi sedini mungkin dan selalu melatih ROM secara rutin.
