Berdasarkan status gizi dari 29 responden dengan gizi normal terdapat 11 orang (38%) mempunyai Hb nya rendah, sedangkan dari 5 responden gizi lebih terdapat 1 orang (20%) kadar Hb rendah. Disarankan untuk lansia agar menjaga pola hidup sehat, mengkonsumsi makanan bergizi dan rajin kontrol untuk berobat guna untuk mengontrol kadar Hb yang rendah bagi lansia yang sudah terdapat anemia. Berdasarkan data dari seluruh Rumah Sakit Jiwa di Indonesia sekitar 70% halusinasi yang dialami oleh pasien adalah halusinasi pendengaran. Penelitian ini bertujuan untuk memperoleh gambaran dan pengalaman belajar dalam penerapan dan penatalaksanaan Asuhan Keperawatan pada Pasien Skizofrenia dengan masalah Keperawatan Gangguan Persepsi Sensori: Halusinasi Pendengaran di Rumah Sakit Jiwa Ernaldi Bahar Palembang Tahun 2019. Jenis penelitian ini adalah deskriptif dalam bentuk studi kasus menggunakan pendekatan proses keperawatan yang meliputi pengkajian, diagnosis keperawatan, perencanaan, pelaksanaan, dan evaluasi dengan subyek penelitian berjumlah dua pasien skizofrenia dengan masalah keperawatan gangguan persepsi sensori: halusinasi pendengaran di ruang merpati Rumah Sakit Jiwa Ernaldi Bahar Palembang. Dilaksanakan selama tujuh hari pada tanggal 12-19 April 2019. Hasil pengkajian didapatkan data yang menjadi focus dalam masalah keperawatan gangguan persepsi sensori: halusinasi pendengaran yaitu pasien mengatakan mendengar suara-suara orang yang mengajaknya berbicara dan menyuruhnya untuk melakukan sesuatu, suara itu muncul secara tiba-tiba ketika pasien sedang melamun, sendirian atau tidak beraktivitas. Diagnosa keperawatan yang didapatkan ada lima, yaitu diagnosa yang menjadi penyebab adalah gangguan konsep diri: harga diri rendah dan isolasi social, diagnosa utama adalah gangguan persepsi sensori: halusinasi pendengaran, dan diagnosa yang merupakan dampak adalah waham curiga dan resiko perilaku kekerasan. Intervensi disusun menggunakan strategi pelaksanaan (SP), dengan tujuan halusinasi pendengaran pasien dapat terkontrol. Implementasi dilakukan sesuai dengan intervensi yang telah disusun, yaitu dengan membina hubungan saling percaya, melakukan strategi pelaksanaan dari SP I, mengontrol halusinasi dengan cara menghardik halusinasi, SP II, mengontrol halusinasi dengan cara bercakap-cakap dengan orang lain, SP III, mengontrol halusinasi dengan cara melakukan kegiatan terjadwal, dan SP IV, mengontrol halusinasi dengan cara minum obat teratur. Setelah dilakukan asuhan keperawatan selama tujuh hari, didapatkan halusinasi pendengaran pasien dapat terkontrol dengan cara menghardik halusinasi, bercakap-cakap dengan orang lain, melakukan kegiatan terjadwal dan minum obat teratur. Kerjasama antara tim kesehatan khususnya perawat sebagai care giver dengan pasien atau keluarga pasien sangat diperlukan untuk keberhasilan asuhan keperawatan pada pasien, peran keluarga juga sangat penting dalam merawat pasien skizofrenia dengan gangguan persepsi sensori: halusinasi pendengaran. Diharapkan perawat praktisi dapat memberikan pelayanan keperawatan bagi pasien skizofrenia yang telah pulang dengan melakukan kunjungan rumah atau home care dan memotivasi keluarga untuk selalu mengontrol dan mengawasi pasien. Description An account of the resource Bayi Berat Lahir Rendah (BBLR) adalah bayi dengan berat lahir kurang dari 2500 gram tanpa memandang masa gestasi, angka kejadian BBLR di Indonesia dari tahun 2007 hingga tahun 2013 terjadi penurunan, namun lambat dalam 7 tahun terakhir. Masalah yang terjadi pada BBLR yaitu ketidakseimbangan Nutrisi Kurang dari Kebutuhan Tubuh. Pada masa neonatus, nutrisi BBLR merupakan nutrisi paling tepat dalam hal pemenuhan nutrisi bagi BBLR. Desain penelitian ini menggunakan metode sampling. Metode sampling yang digunakan adalah Purposive Sampling. Sampel yang diambil sebanyak 2 responden yaitu pasien dengan Bayi Berat Badan Lahir Rendah dengan masalah ketidakseimbangan Nutrisi Kurang dari kebutuhan tubuh di RS TK.II dr AK. Ghani Palembang Tahun 2019. Hasil penelitian menunjukkan setelah dilakukan implementasi keperawatan pemberian ASI air susu ibu pada bayi BBLR, mengkaji refleks hisap, menimbang berat badan bayi setiap hari selama tiga hari berat badan bayi mengalami peningkatan. Melihat hasil penelitian ini maka di harapkan agar kedua ibu dari masing-masing bayi dapat memberikan ASI secara aktif guna memenuhi nutrisi untuk tumbuh kembangnya.
